Job Description of Basic Home Care Giver I. Patient Nursing Care 1) Bed making a) Using washed clean linens to make patient's bed at least twice a day. b) In case of bed wetting or soiling of bed linens, linens will be changed immediately. c) It doesn’t include laundry or cleaning patient's personal clothes or linens. Such clothes will be kept at designated places. 2) Oral Care a) Independent** patients, assistance in using brush for oral care at least twice a day. b) Dependent** patients, oral care using mouth rinse and artery forceps at least twice daily. c) Oral rinse using plain water after every feed through mouth. 3) Skin care a) Bath i) Towel and water bath in the bed daily once or as decided by the medical team for bed ridden and bed bound patients. ii) Hair wash in the bed as decided by the medical team. iii) Chair bath in the bathroom for ambulant** alert patient. b) Skin Moisturisation as and when needed c) Nail and Hair i) Periodic manicure and pedicure. ii) Regular grooming and shaving which does not include hair cut. d) Clothing i) Putting washed and clean clothes and changing clothes as and when required. e) Minor Dressing of wounds and ulcers** 4) Feeding a) Oral feeding i) Serving food in plate and filling water in cup. ii) Cleaning patient's plate, spoons, and cup after feed. iii) Assistance in feeding either hand feeding or through spoon in dependent patients. b) Riles tube/ PEG feeding i) Checking position of tube before feed. ii) Tube feeding of prepared food. 5*. Preparation of tea/coffee/milk/ protein mix/ only for patient consumption* 6. Bowel and Bladder Care a) Bowel Care i. Assistance in getting to and from the toilet (sitting in commode). ii. Cleaning up the patient after going to toilet. iii. Keeping, disposing, cleaning bed pan for dependent patients. iv. Changing diaper and clean up as and when required. v. Diaper will be disposed at designated place as advised by the family. b) Bladder Care i. Assistance in getting to and from the toilet (sitting in commode). ii. Keeping, disposing and cleaning the urinal/ Uro Bag as and when required. iii. Condom catheter- putting condom drainage, checking position daily. iv. Foley's catheter- Maintaining the cleanliness of catheter. v. They will not insert a Foley’s catheter or do the bladder wash. 7. Mobility Assistance: i. Turning the patient in case of dependent or bedridden as per the care plan. ii. Assisting the patient in ambulation as per care plan. iii. Performing Passive exercise as per the care plan. 8. Medication Management: i. Monitoring, Supervising or Assistance in giving Oral medicines as per prescription. ii. Monitoring the IV infusions (not initiating) . iii. Administering subcutaneous medicine as recommended by the GWEC physician. II. Assessment and Reporting: a) Checking Vitals:(Pulse, Respiratory Rate, Bp, Temp) as per medical team advise. b) Random Blood Sugar through glucometer as and when required. c) Regular monitoring of general condition and escalating according to the severity. d) Daily Inspecting for the signs of bedsores. e) Maintaining Input-output Chart, Diet Chart, Medication Chart. f) Initiating the emergency protocol as and when needed. III. Emergency Protocol a) In case of any emergency or medical condition that cannot be handled by the basic home care giver, the issue will be escalated to the clinic nurse. b) In case the patient needs to be shifted to nearest hospital, the responsibility of basic home care giver is to inform the family about the clinical condition. Grand World Clinics wouldn’t be held responsible for any delay in transporting to hospital. c) Visit by the registered nurse or the medical team will be solely at the discretion of Grand World Elder Care Clinics. * Assessment and monitoring may vary from patients to patients ** Nature of the wound would be decided by the medical team ** Level of dependence will be decided by the medical team
